Abstrak
This study focuses on efforts to reduce the risk of work accidents at PT Chroma International using FMEA and FTA methods. The purpose of this study is to analyze the potential for priority work accidents that occur in wind tunnel production using the FMEA (Failure Mode and Effect Analysis) method, to analyze the causes Work accidents are prioritized in order to take preventive measures using the FTA (Fault Tree Analysis) method. The methods used are the FMEA (Failure Mode and Effect Analysis) method and the FTA (Fault Tree Analysis) method. After conducting an analysis using the FMEA (Failure Mode and Effect Analysis) method on accidents that occurred at PT Chroma International, the potential for priority work accidents was obtained, namely hands and feet being hit by grinders with an RPN (Risk Priority Number) value of 112.71. After analyzing using FTA (Fault Tree Analysis) so that 13 causes of priority accidents were obtained, namely uncomfortable using PPE ( Personal Protective Equipment), lack of coordination, less well , lack of concentration, lack of enthusiasm, excessive joking, not paying attention to the situation, limited workspace, slippery work floor , messy work environment, PPE ( Personal Protective Equipment) unavailable, lack of attention and lack of experience
